Vidant Medical Group and Vidant Edgecombe Hospital in Tarboro, NC are seeking a Nurse Practitioner or Physician Assistant to join their Hospice and/or Palliative Care team. Applicants should have a minimum of 2 years of direct patient care experience. Candidates with hospice and/or palliative care are preferred.

Highlights include:

• Primarily M-F shifts, 8am – 5pm with some evening and weekend activities
• Continuity care coordination for Inpatient and home-based patients
• Multidisciplinary care team approach
• Model has a strong focus on patient and family-centered care

Duties include:

• Perform consultation across hospital
• Perform daily clinical rounds on inpatient unit
• Provide clinical support and education for staff and providers
• Direct/participate in family conferences and interdisciplinary team meetings
• Perform and documents face to face assessments
• Participate in Quality Improvement and Patient/Family Experience activities
• Participate in developing palliative care services across hospital

Currently undergoing expansion and renovation, the new center is anticipated to open in the summer of 2018.
