Palliative Medicine Advanced Practice Provider (NP/PA)
Location: Phoenix, AZ
Department: Hospice & Palliative Care
Employment Type: Full Time, permanent position
Setting: Inpatient and Outpatient
Position Overview
This Advanced Practice Provider role focuses on improving quality of life for patients with serious, life-limiting illness through expert symptom management, psychosocial support, and shared medical decision-making. The APP functions as part of a multidisciplinary palliative care team providing care across inpatient and outpatient settings.
Position Highlights
* Multidisciplinary palliative care practice
* Combined outpatient clinic and inpatient consult service
* Focus on symptom management, goals-of-care discussions, and advanced care planning

* Leadership, education, and quality improvement opportunities
* Structured weekday outpatient schedule with inpatient service coverage as assigned
Clinical Responsibilities
* Perform comprehensive assessments of patients with serious or life-limiting illness
* Manage complex symptoms including pain, dyspnea, nausea, anxiety, and delirium
* Facilitate shared medical decision-making and advance care planning
* Order, conduct, and interpret diagnostic tests as clinically indicated
* Prescribe pharmacologic and non-pharmacologic therapies
* Provide education and counseling to patients, families, and caregivers
* Coordinate care across the continuum with interdisciplinary team members
* Make appropriate referrals and monitor effectiveness of therapeutic interventions
* Perform minor procedures as approved by regulatory scope of practice
Practice Environment
* Outpatient clinic schedule Monday through Friday, 8:00 a.m. to 5:00 p.m.
* Inpatient service coverage provided in 7-day blocks when assigned
* Weekend coverage as needed for inpatient service
Professional Expectations
* Demonstrate leadership through committee involvement and team initiatives
* Participate in education, research, and performance improvement activities
* Engage in professional organizations and community activities
* Practice in accordance with professional standards and ethical guidelines
Qualifications
* DEA registration required
* Active BLS certification required
Nurse Practitioner
* Master’s-prepared graduate of an accredited Nurse Practitioner program
* Current Arizona RN and APRN license
* Certification as one of the following:
* Family Nurse Practitioner (FNP)
* Adult Nurse Practitioner (ANP)
* Acute Care Nurse Practitioner (ACNP)
Physician Assistant
* Graduate of an accredited Physician Assistant program
* Current NCCPA certification
* Registration with the Arizona State Board of Medical Practice
