Position Type: Full-Time | Skilled Nursing Facility Setting
Compensation: $107,488 – $131,860 annually (no cap on productivity income potential)
About the Opportunity:
Join a leading post-acute and palliative care organization as a full-time Nurse Practitioner (NP) or Physician Assistant (PA) providing palliative services at skilled nursing facilities near Philadelphia, Pennsylvania. This is an opportunity to deliver meaningful, patient-centered care while working alongside a compassionate and experienced interdisciplinary team.
Responsibilities:
Provide comprehensive palliative care to residents in skilled nursing facilities
Collaborate with physicians, nurses, social workers, and facility staff to optimize patient outcomes
Assess, diagnose, and manage symptoms to improve quality of life and patient comfort
Develop individualized care plans with patients and families
Maintain thorough and accurate documentation
Qualifications:
Current and active Pennsylvania NP or PA license
Board certification as an NP or PA required
Experience in palliative care, geriatrics, or post-acute care preferred
Strong communication, collaboration, and clinical assessment skills
