**Palliative Care PA/NP Opening in Burlington, MA**

**Job Details:**
- Full-time, permanent position
- The Division of Palliative Care and Geriatric Medicine seeks a full-time advance practitioner (NP or PA) to join its expanding clinical service.
- This positions responsibilities include providing comprehensive outpatient palliative care services to seriously ill patients in multiple specialty clinics, including oncology, pulmonary, nephrology, neurology and cardiology. The advance practitioner will serve as part of an interdisciplinary team including physicians, social workers and a pharmacist.
**Job Duties:**
- Evaluate and address patients needs including: pain and symptom management, goal setting and clarification of treatment options, advanced care planning and discussion regarding illness and end of life issues, in conjunction with other health professionals.
- Provide patient education to the patients and their family according to the plan of care to facilitate optimal patient and family support relative to the patient's illness.
- Work effectively with other health care professionals
- Consult with palliative care physician as needed, informs consulting team of recommended interventions and collaborates with other physicians as needed.
- Attend and participate in Division of Palliative Care team meetings.
- Prepare and maintain accurate patient records
- Assist in development of clinical practice guidelines/standards in support of quality palliative care as requested by supervising physician
- Participate in division on-call rotation.
**Qualifications include:**
- Minimum five years of patient care experience
- Patient care experience in one or more of the following disciplines: oncology, pulmonary, nephrology, neurology, cardiology, geriatrics
- Licensed to practice in Massachusetts (or eligible to obtain)
- Active DEA registration
