Transitional Care Management GNP/PA
2+ years experience working specifically with geriatric population
The Assessment Center provides older adults with a comprehensive, interdisciplinary team evaluation to help the patient, family, and primary physician develop a treatment plan that addresses medical, psychological, social, and functional issues.
Common issues addressed at the GAC include: Chronic heart or lung disease Chronic pain Dementia and memory loss Depression Diabetes Falls Frailty Multiple medications Urinary incontinence We manage the problems of the elderly including: early memory loss, gait instability, dementia disorders (including Alzheimer's disease), behavioral abnormalities, urinary incontinence and other complex chronic conditions.
For the caregiver, we offer supportive counseling, education, and resource referrals
