Join a collaborative clinical team providing hands-on geriatric care in a skilled nursing setting. This role focuses on managing acute and chronic conditions while delivering compassionate, patient-centered care to residents and their families.
Job Responsibilities
• 👩⚕️ Perform patient histories and physical exams
• 🧪 Order, interpret, and document diagnostic studies
• 💊 Prescribe medications and therapeutic treatments
• 🧠 Manage acute & chronic geriatric conditions using evidence-based care
• 🤝 Coordinate referrals and collaborate with multidisciplinary teams
• 📚 Educate residents and families on care plans and health maintenance
• 📝 Maintain accurate and timely clinical documentation
Qualifications
• NP or PA certification
• Active or eligible state license
• Geriatric or SNF experience preferred
Working Hours
