Advanced Practice Provider – Geriatrics Location: Norwich, CT Shift Detail: Monday–Friday Overview: An excellent opportunity for a Nurse Practitioner or Physician Assistant to join a growing geriatrics team dedicated to providing compassionate, value-based care to older adults in Eastern Connecticut. This position focuses on delivering care to patients in home-based and community settings, such as Assisted Living and Congregate Housing facilities. Position Highlights: • Provide comprehensive geriatric care through a home-based care model designed to enhance quality of life and independence for older adults. • Collaborate with a multidisciplinary team of healthcare professionals to coordinate and manage patient care. • Full-time, weekday schedule with flexibility in locations based on the candidate’s home base. • Meaningful and rewarding work environment focused on improving outcomes for elderly patients. Role: • Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA). • Setting: Geriatric Medicine – home-based and community settings. • Schedule: Full-time, weekdays. Qualifications: • Unrestricted State of Connecticut medical license (or eligibility to obtain). • Training as an APRN (Family, Adult, or Geriatric Nurse Practitioner) or Physician Assistant. • Minimum of two years of post-graduate experience as an APRN or PA preferred. • New graduates with relevant nursing or geriatrics experience are encouraged to apply.
