Advanced Practice Provider – Geriatrics Location: Norwich, CT Shift Detail: Monday–Friday Overview: An excellent opportunity for a Nurse Practitioner or Physician Assistant to join a growing geriatrics team dedicated to providing compassionate, value-based care to older adults in Eastern Connecticut. This position focuses on delivering care to patients in home-based and community settings, such as Assisted Living and Congregate Housing facilities. Role: • Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA). • Setting: Geriatric Medicine – home-based and community settings. • Schedule: Full-time, weekdays. Qualifications: • Unrestricted State of Connecticut medical license (or eligibility to obtain). • Training as an APRN (Family, Adult, or Geriatric Nurse Practitioner) or Physician Assistant. • Minimum of two years of post-graduate experience as an APRN or PA preferred. • New graduates with relevant nursing or geriatrics experience are encouraged to apply.
