The Float Provider will travel approximately 3 weeks out of every month, providing coverage at SNFs in various locations throughout the state of Ohio. All travel expenses are covered by the company.


COMPANY PROFILE:
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies.


POSITION DESCRIPTION:
The Float Provider will provide as-needed coverage to geriatric patients in SNFs. The Float Provider will not be permanently assigned to one facility, but will float within the designated market for purposes of temporarily filling vacancies, sick leave coverage, etc. The exact travel parameters will be mutually agreed to by the company and the provider at the time of hire.

The Float Provider will be responsible for:

- effectively medically managing enrolled SNF patients
- conducting comprehensive assessments
- developing and delivering individualized plans of care
- attending facility care team meetings
- communicating with family members and caregivers
- ensuring the care team understands the member’s goals of care and needs

As part of the clinical team, the Float Provider will be supported by the Clinical Manager and/or Director, as well as the local Medical Director.

The organization uses an EHR.

The territory for the Float Provider includes the above-listed locations. The Float Provider should expect to travel approximately three (3) weeks per month. All travel expenses are covered by the employer.


REQUIREMENTS:
- Ohio NP or PA license
- DEA
- 1+ year advanced practice experience
- 1+ year geriatric care experience
