- Full-Time/ Permanent Role
- Mon-Fri 8am- 5pm - some shared call
- 100% outpatient
- Duties include; conduct home and face to face visits in SNF & ALFs within a 20 mile radius providing end of life care
- NC license in hand or in the process
- Ideal candidate will have 2 years of Hospice or Palliative care experience
