Physician Assistant – Primary Care, MAT & Mobile Health

Location: East Los Angeles, California
Salary: $140,000–$160,000 annually
Schedule: Full-time

A mission-driven, nonprofit community health organization in Los Angeles is seeking a bilingual Physician Assistant to join its interdisciplinary primary care team. This position offers the opportunity to provide meaningful, patient-centered care to culturally diverse and medically underserved communities across multiple outpatient locations and through a mobile health program.

The organization is a Federally Qualified Health Center offering integrated medical, pediatric, behavioral health, women’s health, dental, pharmacy and supportive community services. Its patient-centered approach emphasizes access, prevention, chronic disease management and addressing the medical and social factors that affect long-term health.

This is an excellent opportunity for a Physician Assistant who enjoys variety, community-based medicine and caring for patients across the lifespan. The incoming provider will serve as a float clinician throughout East Los Angeles and provide services through the organization’s Mobile Health Unit. Experience treating substance use disorders and providing Medication-Assisted Treatment is required.

Responsibilities

• Obtain and document comprehensive medical and social histories
• Perform physical examinations and clinical assessments
• Diagnose and manage acute and chronic medical conditions
• Develop individualized, evidence-based treatment plans
• Order and interpret laboratory testing, imaging and other diagnostic studies
• Provide preventive care, health screenings, wellness education and counseling
• Manage chronic conditions and support patients with long-term self-management
• Provide care to patients across the lifespan, including pediatric and adult populations
• Evaluate and treat patients with substance use disorders
• Provide Medication-Assisted Treatment and ongoing SUD care management
• Prescribe medications in accordance with applicable laws, regulations and clinical protocols
• Collaborate with physicians and other members of the interdisciplinary healthcare team
• Refer patients to specialty care, behavioral health, social services and community resources
• Conduct pre-visit planning and review relevant laboratory, imaging, consultation and discharge records
• Provide float coverage at outpatient clinics and through the Mobile Health Unit
• Support culturally and linguistically appropriate patient education and communication
• Participate in team huddles, case conferences, departmental meetings and care-management activities
• Assist with quality-improvement initiatives, staff education, data collection and program evaluation
• Complete clinical documentation, coding and billing requirements accurately and promptly
• Provide appropriate oversight and guidance to medical assistants, community health workers and other assigned team members

Qualifications

• Graduate of an accredited Physician Assistant program
• Current or eligible California Physician Assistant license
• Current certification as a Physician Assistant
• Experience providing Medication-Assisted Treatment for substance use disorders
• Experience assessing, treating and managing patients with substance use disorders
• Pediatric care experience preferred
• Bilingual fluency in English and Spanish required
• Ability to provide compassionate care to culturally and socioeconomically diverse populations
• Strong clinical judgment, communication and interdisciplinary collaboration skills
• Comfortable traveling between outpatient locations and working in a mobile health setting
• Valid California driver’s license
• Reliable automobile and required automobile insurance coverage
• Ability to successfully complete required background, physical and tuberculosis clearances
