Seeking Nurse Practitioner or Physician Assistant to join post-acute care team in Brooklyn, New York
Position Highlights:
Full-time schedule, 4-5 days per week

Responsibilities:
Perform comprehensive patient assessments, including histories and physical examinations
Develop, implement, and adjust individualized care plans
Monitor patient progress and coordinate care across disciplines
Collaborate with physicians, nursing staff, therapists, and other healthcare professionals
Educate patients and families regarding disease management and preventive care
Maintain accurate, timely, and compliant medical documentation
Deliver high-quality, patient-centered post-acute care
Qualifications:
Nurse Practitioner or Physician Assistant certification
Active New York NP or PA license
Active DEA or ability to obtain
Experience in post-acute, acute care, emergency medicine, or outpatient practice preferred
New graduates encouraged to apply
Strong communication, clinical assessment, and teamwork skills
Compensation & Benefits:
Estimated compensation range: $113,000 - $135,000 annually
Fee-for-service compensation model
