Job description Post-Acute Care NP/PA Opening Brooklyn, NY Full Time Permanent Position Seeking Nurse Practitioner or Physician Assistant to join post-acute care team in Brooklyn, New York Position Highlights: Full-time schedule, 4-5 days per week Skilled nursing facility-based practice Comprehensive 3-month onboarding program Dedicated clinical field mentor and ongoing support from experienced post-acute medicine leaders Strong clinical and operational leadership support Growth and leadership opportunities available within the market Participation in innovative population health and value-based care initiatives Access to TeamACO Medicare Shared Savings Program opportunities Medical Director Leadership Academy available for professional development and leadership growth Collaborative multidisciplinary care environment No cap on productivity-based income potential Responsibilities: Perform comprehensive patient assessments, including histories and physical examinations Develop, implement, and adjust individualized care plans Monitor patient progress and coordinate care across disciplines Collaborate with physicians, nursing staff, therapists, and other healthcare professionals Educate patients and families regarding disease management and preventive care Maintain accurate, timely, and compliant medical documentation Deliver high-quality, patient-centered post-acute care Qualifications: Nurse Practitioner or Physician Assistant certification Active New York NP or PA license Active DEA or ability to obtain Experience in post-acute, acute care, emergency medicine, or outpatient practice preferred New graduates encouraged to apply Strong communication, clinical assessment, and teamwork skills Compensation & Benefits: Estimated compensation range: $113,000 - $135,000 annually Fee-for-service compensation model Uncapped productivity earning potential Medical, dental, and vision insurance Retirement plan options Paid time off Professional development and leadership training opportunities Potential participation in Medicare Shared Savings Program incentives Community Highlights: Practice located in Brooklyn, New York Diverse patient population and strong community presence Access to world-class dining, entertainment, and cultural attractions Convenient public transportation and regional accessibility Opportunity to practice in one of the nation's most dynamic healthcare markets Estimated pay range/amount is $120k to $160k.
