Looking for a consistent part-time locums opportunity? A Certified Community Behavioral Health Clinic in the Los Angeles area is seeking an experienced Nurse Practitioner or Physician Assistant to provide outpatient primary care within an integrated care setting.
Details/Requirements:
--Start: ASAP and ongoing--Only 2 days/16 hours per week
--Schedule: Monday and Wednesday, 8am-5pm
--Outpatient setting
--Nurse Practitioners and Physician Assistants considered
--Provide primary care screenings and monitor chronic health conditions
--Support integrated care planning alongside the behavioral health team
--Strong chronic disease management experience preferred
--Behavioral health and/or SPMI population experience preferred
--This is a great opportunity for an NP or PA looking to add a predictable two-day-per-week assignment while working with an integrated care team serving a unique patient population.
