Key Responsibilities:
Conduct thorough patient assessments, including well visits, sick visits, and chronic care management
Develop and manage individualized care plans for patients
Prescribe and manage medications in accordance with clinical guidelines
Educate patients on disease prevention, wellness, and chronic condition management
Perform diagnostic testing and interpret results to guide treatment decisions
Maintain accurate and timely electronic health records (EHR)
Collaborate with physicians, nurses, and other healthcare team members to ensure cohesive patient care
Adapt to flexible scheduling to meet patient needs across a 4-day or 5-day work week
Qualifications:
Graduate of an accredited Physician Assistant program
Certification as a Physician Assistant
Active Arizona state licensure to practice as a PA
Experience or training in chronic disease management, well visits, and medication management
Competence with diagnostic testing and interpretation of results
Proficiency using Electronic Health Records (EHR)
Strong communication, patient education, and teamwork skills
Ability to adapt to flexible schedules and manage time efficiently
