UNC Health Medical Group is looking for a Per Diem Advanced Practice Provider (NP/PA) to join the growing UNC Waynes’s Post Discharge Transitions of Care Clinic as a virtual APP. If this is the position for you, please apply!

Responsibilities:
- Provide remote patient management and care, collaborating with healthcare providers.
- Utilize electronic medical records (EMR) systems to facilitate communication and care delivery.
- Manage patient communication, including scheduling appointments and sending reminders.
- Facilitate video consultations and ensure timely responses to patient inquiries.
(Two or more days a month)
Hours: 8:00 a.m. - 4:00 p.m.
EMR:EPIC
Qualifications:
- Graduation from an accredited NP or PA school
- One or more years of full panel primary care experienced required
- North Carolina licensed
- BLS & ACLS certified
