Seeking a Nurse Practitioner or Physician Assistant with a Master’s degree from an accredited Program
Two plus years’ experience as a Nurse Practitioner or Physician’s Assistant highly desirable; minimum of two years of experience in a community health
- Active Nurse Practitioner or Physician Assistant License in the state of California;
- DEA License;
- Current BLS certificate
- Active NPI
- Provide high-quality, patient-centered and values-based primary medical care.
- Effectively use huddles, care gap reports, and integration work to meet treatment and preventative care measures.
- Provide direct education to patients regarding their medical conditions and treatment options.
- Appropriate and complete referrals to specialty care as needed.
- Prescribe and/or dispense medications as appropriate.
- Efficiently and timely manage and respond to requests for medication refills, document completion, patient messages and general questions.
- Collaborate with behavioral health, dental, optometry and other health care team members as appropriate for integration, consultation, referral and follow-up.
- Assist the Chief Medical Officer and Medical Director in program development, implementation and evaluation as necessary.
- Active participation in the health center’s quality assurance and peer-review programs.
- Provide primary medical care services in accordance with health center policies and board approved privileging.
- Provide coverage to the health center’s urgent care schedules.
- Practice in an efficient and cost effective manner; appropriately order labs and tests.
- Provide on-call coverage for after-hours care in accordance with health center protocols.
- Lead the care team in problem-solving to see late arriving patients and patients with urgent needs.
- Collaborate with the medical management team to maximize patient appointment access.
