Overview:
We are seeking a dedicated and skilled Physician Assistant to join our team in Albany, NY. The ideal candidate will have experience in skilled nursing facilities and will be responsible for providing high-quality care to our patients. This role involves managing admissions, discharges, and chronic care management, ensuring comprehensive and continuous care.

Key Responsibilities:
- Conduct thorough assessments of patients upon admission and develop individualized care plans
- Manage patient discharges, ensuring all necessary documentation is completed and communicated effectively with the care team
- Provide ongoing management of chronic health conditions, working closely with patients and families to optimize health outcomes
- Collaborate with physicians, specialists, and other healthcare professionals to deliver coordinated care
- Maintain accurate and up-to-date patient records and documentation
- Educate patients and families about health conditions, treatment plans, and preventive care strategies
- Participate in quality improvement initiatives and contribute to a positive work environment

Qualifications:
- Valid Physician Assistant license in the state of New York
- Minimum of 2 years of experience in a skilled nursing facility or similar setting
- Strong clinical skills in managing admissions, discharges, and chronic care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Strong organizational and time management abilities
