Family Nurse Practitioner Opening – Hammond, IN Located in Hammond, Indiana Full-time, exempt Nurse Practitioner opportunity providing medical services to patients of all ages within scope of licensure, training, and credentialing under physician supervision. Position Overview -Full-time exempt position -Provide care to patients of all ages -Work under supervision of collaborating physician -Practice within scope of licensing, specialty, and credentialing -Must meet organizational credentialing requirements -Must demonstrate clinical competency in area of medicine -Team-based, collaborative care model -Office-based clinical environment Essential Clinical Responsibilities -Obtain relevant health and medical histories -Identify medical risks and patient needs -Update and document changes in health status -Order appropriate diagnostic testing -Develop and implement treatment plans of care -Prescribe appropriate pharmacologic therapies -Prescribe non-pharmacologic interventions -Develop and provide patient education plans -Make referrals to specialists and community agencies as appropriate -Schedule follow-up appointments per best practice standards -Document patient outcomes to evaluate effectiveness of care plans -Reassess and modify treatment plans as necessary to achieve goals -Provide education to promote optimal health -Incorporate patient input into treatment planning -Support appropriate utilization of healthcare resources -Complete charting and task responses in a timely manner to avoid delays in care -Participate in quality assurance and peer review activities -Prepare required reports and documentation -Attend required meetings and training sessions -Collaborate with multidisciplinary care professionals to provide comprehensive care -Perform other assigned duties within scope as directed Competencies -Planning and strategic foresight -Responsible decision making -Integrity and accountability -Innovation and creativity -Adaptability and flexibility -Leadership, teamwork, and conflict resolution -Professionalism and strong work ethic -Empathy and patient-centered care Qualifications -Must be at least 18 years of age -Must have reliable transportation -Valid RN license -Valid APRN license -Valid CSR license -Valid CRS certification -Valid federal DEA license -Preferred: Two years of healthcare experience -Minimum: Bachelor’s degree in Nursing -Preferred: Master’s degree in Nursing Required Skills -Ability to analyze and solve problems at strategic and tactical levels -Strong interpersonal and customer service skills -Ability to interpret and apply applicable laws, regulations, and policies -Understanding of organizational hierarchy and administrative practices -Strong organizational and planning skills -Critical thinking and problem solving -Emotional intelligence and composure under pressure -Ethical reasoning and decision-making -Strong attention to detail -Receptive and responsive to feedback -Excellent verbal and written communication skills -Time management and prioritization skills -Proficiency with Microsoft Office Suite or related software Work Environment -Office-based setting -Frequent in-person and telephone contact with patients and healthcare personnel -High-interaction, interruptive environment -Work may be stressful at times -Travel outside primary locations is rare Physical Requirements -Prolonged sitting and standing -May work shifts longer than eight hours -May work day, evening, or weekend shifts -Frequent reaching, stooping, kneeling, crouching, talking, and hearing -Occasional lifting up to 50 lbs -Vision requirements include close, distance, color, peripheral, depth perception, and focus adjustment -Reasonable accommodation available for individuals with disabilities This is an excellent opportunity for a Nurse Practitioner seeking a collaborative clinical environment with comprehensive patient care responsibilities across the lifespan.
