We are seeking an experienced Family Practice Nurse Practitioner to provide locum services in Barlow, OR. Coverage is needed ASAPand ongoing for 3 months with the option to extend.
Job Details:
- Setting: Telehealth/Home Health
- Shift/Schedule Details:
- Full-time (40 hours/week) Monday–Friday or Part-time available (20 hours)
- 8 am-5 pm with a 1-hour lunch
- Skills/Procedures/Required Case Types:
- Conduct 5–6 in-home health assessments per day
- Perform annual risk assessments and chronic condition reviews
- Assessment-only role — no treatment or prescribing required
- Escalate urgent findings to the patient’s PCP
- A1C testing only (if applicable; no additional screenings required)
Requirements:
- State License: OR
