Bronx-Based House Call Physician Assistant Opportunity
Location: Bronx base, New York, NY 10001 (House calls across NYC area)
Position: Physician Assistant - Family Practice (1 opening)
Deliver personalized home-based primary care from a Bronx hub, focusing on homebound patients throughout NYC.
Key Responsibilities:
- Conduct house calls for exams, chronic care, med recs, and acute visits
- Order labs, interpret results, and coordinate with specialists/pharmacies
- Manage EHR documentation, preventive screenings, and transitions of care
- Provide end-of-life support and family education during visits
Qualifications:
- NCCPA-certified PA with active NY license
- Family practice or home health experience preferred
- Reliable vehicle, BLS; comfort with geriatric/urban patients
