Position Overview
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality outpatient primary care services with a focus on Hormone Replacement Therapy (HRT). This role serves a diverse patient population, including geriatrics, in a collaborative and patient-centered environment.
The ideal candidate has a strong background in internal medicine or primary care, experience with hormone therapy, and a passion for wellness, preventative care, and long-term patient management.
Key Responsibilities

Deliver comprehensive outpatient primary care and internal medicine services

Conduct patient evaluations, diagnostic assessments, and develop treatment plans

Manage chronic conditions and support longitudinal patient care

Monitor patient progress and adjust treatment plans as clinically appropriate

Provide education on wellness, preventative care, weight management, and lifestyle modifications

Collaborate closely with physicians and clinical staff to optimize patient outcomes

Maintain accurate, timely documentation using Athenahealth (Athena) or similar EHR systems

Ensure compliance with HIPAA and all applicable clinical and regulatory standards
Patient & Practice Details

Patient Volume: 30-35 patients per day

Care Mix: Approximately 1/3 telemedicine, remainder in-clinic

Patient Age Range: 12 to 95 years

Practice Focus: Outpatient primary care with hormone replacement therapy
Qualifications
-
Master's Degree as a Nurse Practitioner or Physician Assistant
-
Active, unrestricted Texas NP or PA license
-
PA-C or FNP-C certification required
-
Active DEA license
-
Minimum 2 years of clinical experience (new graduates not considered)

Experience in outpatient clinics, internal medicine, primary care, or post-acute care

Required experience in Hormone Replacement Therapy

Background in wellness, weight management, and preventative medicine preferred

Experience treating geriatric populations
-
Proficiency with Athenahealth (Athena) or similar EHR systems
-
Strong knowledge of diagnostic evaluation, anatomy and physiology, medical terminology, and ICD-10 coding
