Seeking a Nurse Practitioner or Physician Assistant to join a post-acute care team in the Syracuse, New York area. This full-time opportunity involves rounding in skilled nursing facilities with strong clinical support, training, autonomy, and earning potential.
Position Highlights:
Full-time schedule, five days per week
Rounding in skilled nursing facilities

Responsibilities:
Perform comprehensive patient assessments
Develop and manage individualized care plans

Collaborate with physicians, nursing staff, and healthcare professionals
Educate patients and families on health management and preventive care
Maintain accurate and timely medical records
Qualifications:
Nurse Practitioner or Physician Assistant
Active New York license and DEA
Experience in post-acute, acute, emergency medicine, or clinic settings preferred
New graduates welcome

Compensation & Benefits:
Fee-for-service compensation model
Estimated annual compensation range of $138,334 – $164,001
