Job description APP Family Medicine opening in New Mexico Full time Grants, Nm Summary Primary care provider delivering comprehensive outpatient services to a diverse patient population. Works collaboratively within an interdisciplinary healthcare team that includes nursing staff, medical assistants, care and case managers, behavioral health clinicians, clinical pharmacists, and administrative personnel. The role requires consistent communication, collaboration, and independent clinical judgment to ensure high-quality patient care. Responsibilities * Provide comprehensive medical care for patients with acute and chronic conditions * Exercise independent, professional medical judgment in patient evaluation and management * Order, perform, and interpret diagnostic imaging and laboratory studies * Prescribe medications, treatments, and non-pharmacologic therapies as appropriate * Document all patient encounters and communications accurately and in a timely manner * Establish and manage effective provider–patient relationships * Address and resolve patient complaints professionally and efficiently * Educate and counsel individuals, families, and groups on health-related topics * Serve as a healthcare resource, interdisciplinary consultant, and patient advocate * Work autonomously while collaborating with other healthcare providers and staff as needed * Participate in recruitment, onboarding, and replacement of clinical providers * Contribute to system-wide population health and care management initiatives * Provide input on financial performance, clinical quality, service delivery, budgeting, capital planning, and personnel matters * Contribute clinical expertise to the development, evaluation, and revision of clinical protocols and care processes * Support professional growth through participation in continuing education, literature review, in-services, and workshops * Perform additional duties as assigned to support organizational objectives * Attend and actively participate in scheduled provider, site, and organizational meetings * Adapt assessment techniques based on patient condition, cultural considerations, age, and developmental stage * Maintain coding and encounter documentation standards to ensure completeness, accuracy, timeliness, and appropriate reimbursement
