Family Medicine Advanced Practice Provider – Community Health Setting
Location: Houston, TX
Setting: Outpatient / Primary Care
Employment Type: Full-Time, permanent position
Position Overview
This position provides advanced clinical care in a family medicine setting under the supervision and delegated authority of a physician. The provider delivers comprehensive primary care services across the lifespan, including preventive care, chronic disease management, and acute care visits in a community-based outpatient clinic environment.
Position Highlights
• Provide direct patient care in an outpatient primary care setting
• Diagnose and manage common medical conditions
• Perform wellness visits and preventive care services
• Manage chronic health conditions across all life stages
• Participate in patient education and care planning
• Collaborate with physicians and care team members
• Support quality initiatives and clinical best practices
Primary Responsibilities
• Perform patient assessments and physical examinations
• Conduct wellness visits and health risk assessments
• Develop and implement patient-centered treatment plans
• Order and interpret diagnostic testing
• Provide counseling and education related to diagnosis and care
• Maintain accurate documentation in the electronic health record
• Identify high-risk patients and coordinate referrals as needed
• Support continuity and coordination of care
• Utilize evidence-based practice standards
• Participate in quality improvement and peer review activities
• Assist with development and review of clinical protocols
• Provide indirect supervision of clinical support staff
• Participate in meetings and team functions
• Perform additional duties as assigned
Qualifications
• Graduate of an accredited Physician Assistant program
OR Master of Science in Nursing program
Certifications & Licensure
• Certified by AANP, ANCC, or NCCPA
• Active and unrestricted license in the State of Texas
(Nurse Practitioner or Physician Assistant)
• Current Basic Life Support certification
Skills & Experience
• Knowledge of outpatient primary care principles and treatment standards
• Ability to assess, diagnose, and manage common medical conditions
• Strong documentation and clinical decision-making skills
• Ability to work effectively with diverse patient populations
• Commitment to team-based, patient-centered care
