Overview:
Provide comprehensive primary care services to a manageable patient panel
Average 15–18 patients per day
Monday–Friday schedule supporting strong work-life balance

Responsibilities:
Deliver comprehensive primary care services
Collaborate with interdisciplinary teams to optimize patient outcomes
Ensure smooth transitions of care with hospice, palliative care, and home care agencies
Maintain accurate and timely documentation in the Gehrimed EMR system
Qualifications:
Graduate degree as a Nurse Practitioner or Physician Assistant
Active and unrestricted NP or PA license in New York
Active DEA
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Post-acute care experience preferred but not required (training available)
Compensation:
Estimated annual compensation range: $130,760 – $154,911
No cap on productivity income potential
