APP Family Medicine opening in Auburn, NY Syracuse 28m We are seeking a full-time NP/PA to join our team, providing primary care services in a skilled nursing facility located in Auburn, New York. * Monday - Friday schedule * Providing comprehensive primary care services to a manageable patient panel of 15-18 patients per day * Collaborating with interdisciplinary teams to optimize patient outcomes * Ensuring smooth transitions of care with hospice, palliative care, and home care agencies * Maintaining accurate and timely documentation in our EMR system (Gehrimed) Qualifications * Graduate degree as a nurse practitioner or physician assistant * Active and unrestricted NP or PA license in New York * DEA * Strong clinical assessment and diagnostic skills * Excellent communication and interpersonal abilities * Experience in post-acute care preferred, but not required (training available) Compensation & Benefits * Expected compensation is estimated compensation range of $130,760 to $154,911 with no cap on productivity income potential. * Full benefits including, health insurance, 401k, short and long term disability, flex time (PTO), dental, and vision * Paid professional liability insurance with tail coverage Auburn, NY Auburn is a mid-sized city situated on the tip of Owasco Lake in the finger lakes region of upstate New York.
