- Provider Type Needed: Family Practice Physician Assistant
- Schedule: 8:30 AM – 6:30 PM (30-minute unpaid lunch), four 10-hour shifts per week with a rotation of Tuesday, Wednesday, or Thursday off
Responsibilities and Duties
- Provide outpatient care for scheduled primary care patients and some acute cases as needed
- Manage chronic conditions such as diabetes, hypertension, COPD, asthma, and heart disease
- Address minor injuries, wound care, and pediatric care including immunizations
- Support geriatric care and chronic disease management
- Participate in the management of behavioral health issues, including depression and anxiety
- Work with substance use disorders through the MaT program
- Conduct wellness exams and preventive screenings
- Coordinate referrals and specialty care, including telehealth
- Deliver culturally sensitive care for the Native community
- Educate patients on lifestyle modifications, prevention, and self-management
- Accurately document care in the EHR
- Support after-hours urgent care needs as required
- Collaborate with lab, pharmacy, and other departments for comprehensive follow-up
- Participate in quality improvement initiatives and PCMH standards
Additional Information
- Active Physician Assistant license required in California
- Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certifications required
- Experience with diverse patient populations is preferred
- Immediate coverage need; seeking a candidate ready to step into an established patient panel
