The Position

The Physician Assistant is a vital member of an interdisciplinary healthcare team, contributing to the planning, implementation, and evaluation of individualized care plans. This role provides comprehensive primary care to older adult participants in a variety of settings, including the clinic, patients’ homes, and affiliated long-term care facilities. The Physician Assistant collaborates closely with supervising physicians and the broader care team to ensure the highest quality of care, focusing on both acute and chronic conditions.

Salary and Benefits

  • Salary Range: $106,000- $149,000, Based on Experience

Responsibilities

  • Monday–Friday, 8:00 AM – 4:00 PM
  • Evaluate participants for episodic needs, acute illnesses, and provide follow-up care following hospitalizations
  • Visit nursing facilities to address acute or chronic conditions when participants are unable to visit the clinic
  • Conduct home visits for participants who are homebound due to acute or chronic health concerns
  • Complete initial assessments for all new participants and conduct quarterly evaluations throughout their first year of enrollment
  • Oversee and coordinate care and treatment plans
  • Collaborate as part of the interdisciplinary team to deliver integrated, patient-centered care
  • Participate in phone-only call rotation (1 in 15 weeks)
  • See approximately 10–15 patients per day
    • Limited local travel

Requirements

  • Licensed Physician Assistant in the state of Massachusetts
  • Current NCCPA certification
  • Minimum of one (1) year of recent experience as a Physician Assistant required
  • Experience in primary care, geriatrics or Internal Medicine preferred
  • Current DEA registration in Massachusetts
  • Current CPR Certification
  • COVID-19 Vaccination Required or Exemption
