Dignity Health Medical Group is seeking a dedicated and compassionate Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic team. This unique role offers the exciting opportunity to provide comprehensive primary care to our diverse patient population within our established clinic setting, while also contributing to our cutting-edge Virtualist Program.
The Opportunity: You will play a vital role in our patient care model, dividing your time between direct patient encounters in our clinic and providing virtual care services from a HIPAA-compliant location (e.g., dedicated office space, approved home office). This hybrid approach allows for a broader scope of practice, enhanced work-life balance, and the opportunity to shape the future of healthcare delivery.
Key Responsibilities
Clinic-Based Family Medicine (approx. 50% of time):
- Provide comprehensive primary care to patients of all ages (infants to geriatrics), including preventative care, acute and chronic disease management, and health education.
- Conduct thorough physical examinations, order and interpret diagnostic tests, and develop individualized treatment plans.
- Prescribe medications and treatments in accordance with state regulations and practice protocols.
- Perform minor in-office procedures (e.g., Derm/MSK procedures and Nexplanon]).
- Maintain accurate and timely electronic health records (EHR).
- Collaborate effectively with physicians, nurses, MAs, and other healthcare professionals.
- Participate in team meetings, quality improvement initiatives, and continuing education.
Virtualist Program (approx. 50% of time):
- Must live in the Phoenix, AZ area.
- Provide virtual primary care consultations via secure telehealth platforms (video, phone, secure messaging).
- Assess, diagnose, and treat a wide range of acute and chronic conditions amenable to virtual care.
- Order necessary labs, imaging, and prescriptions remotely.
- Provide appropriate referrals to specialists or in-person care when clinically indicated.
- Educate patients on their health conditions, treatment plans, and self-management strategies.
- Adhere to all telehealth best practices, privacy regulations (HIPAA), and state licensing requirements for virtual care.
- Utilize virtual care tools and technology proficiently.

Job Requirements
Qualifications
- Current and unrestricted licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Arizona.
- Current national board certification (ANCC, AANP, NCCPA).
- BLS certification required.
- Minimum of 3 years of experience as an Advanced Practice Provider in Family Medicine or Primary Care.
- Demonstrate experience or strong interest in telehealth/virtual care delivery.
- Proficiency with Electronic Health Records (EHR) systems; experience with Cerner is a plus.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently and collaboratively within a team environment.
- Strong organizational skills and attention to detail.
- Tech-savvy with the ability to quickly learn and adapt to new technologies.
- Comfortable working with patients virtually and assessing conditions without a direct physical examination when appropriate.

Pay Range
$44.73 - $75.14 /hour
