Responsibilities and Duties
- Conduct comprehensive medical evaluations and develop individualized treatment plans
- Manage chronic and acute conditions including diabetes, hypertension, and infectious diseases
- Administer preventive care services such as immunizations, screenings, and health education
- Provide acute care for common conditions including respiratory infections, skin disorders, and injuries
- Emphasize harm reduction and continuity of care for diverse patient populations
Additional Information
- At least two years of experience in community health, urgent care, or family medicine, preferably with vulnerable populations
- Bilingual (Spanish) skills highly desired
- Experience with Epic EMR strongly preferred
- Must hold a valid California license
