• Shift Schedule: Mon-Fri; 8a-5p (FT)
- 40 hrs per week, 5 days per week
• Patient Volume: At least 18
• Patient Demographics: Adults & Peds
- 12+ months old (required); Infants (preferred)
• Case Mix: Includes some Chronic Pain Management
• Support Staff: 1 RN & 2 MAs
• EMR: Epic
• Other Info: Chart closure within 72 hrs
• Travel, lodging, and malpractice insurance covered
Requirements:
• Active CA License
• BC
• BLS
• DEA
• With 3 yrs of experience in a similar setting
• PAs must have a CA Controlled Substance Education Course Certificate
• NPs must have a Furnishing License with Schedules II
• CVs must have month & date on work experience w/ all gaps in employment accounted for
• Immunizations such as Flu Vaccine, TB Status/X-Ray Results etc
