• Provide exceptional care to patients in a hybrid virtual and in-home health care model
• Part-time, .5 FTE with the ability to go full-time later
• Schedule can be built to work around your needs throughout the day
• Manage a low-volume, higher acuity patient panel
• Provider-empowering organization that helps deliver high value patient care
• Strong base salary range around $75K+ for just 20-25 hours per week
• Full benefits package offered
