#### Position Highlights:
Schedule: Monday through Friday, full-time (no weekends or nights)
Compensation: Fee-for-Service (FFS) model
- *Estimated annual earnings:* $130,000 – $183,000, with no cap on productivity-based income
Employment Model: W-2, benefits-eligible
Responsibilities:
- Provide primary care services in post-acute or outpatient settings
- Conduct evaluations, manage chronic and acute conditions, and coordinate multidisciplinary care
- Collaborate with physicians, facility leadership, and other clinicians
- Build strong relationships with patients, their families, and nursing staff
- Participate in clinical education, quality initiatives, and leadership opportunities

#### Qualifications:
Licensed NP or PA in the State of Michigan (active and unrestricted)
Board Certification in Family Medicine, Internal Medicine, or related field
DEA license (or ability to obtain)
Prior experience in post-acute, long-term care, or primary care preferred
Excellent communication and organizational skills
