APP – Family Medicine Primary Care Location: Norwich, CT Minimum 2yrs Outpatient Primary Care Experience Shift Detail: Monday – Friday, Full-Time Setting: 100% Outpatient Primary Care – Adult Medicine (18+) Position Overview: We are seeking a Family Medicine Nurse Practitioner or Physician Assistant to join our outpatient primary care team in Norwich, CT. From the moment you enter the clinic, you’ll experience a supportive, welcoming environment where teamwork, compassion, and professionalism guide everything we do. Role Scope: • Perform routine physical exams and provide comprehensive primary care services for adults (18+). • Develop and implement medical, educational, and psychosocial care plans. • Collaborate with physicians and care team members to ensure optimal patient outcomes. • Provide continuity of care and patient education. • Manage patient needs within scope of practice, and refer to physicians or specialists as appropriate. Schedule & Call: • Full-time, weekdays only (Monday – Friday). • Flexible scheduling options available. • No call required. Qualifications: • Unrestricted medical license (or eligibility) in the State of Connecticut. • Graduate of an accredited Nurse Practitioner (Family or Adult/Geriatric NP) or Physician Assistant program. • Minimum of 2 years’ experience in primary care or a relevant outpatient setting preferred.
