APP – Family Medicine Primary Care Location: Stamford, CT Minimum 2yrs Outpatient Primary Care Experience Shift Detail: Monday – Friday, Full-Time Setting: 100% Outpatient Primary Care Position Overview: An excellent opportunity is available for a Family Nurse Practitioner or Physician Assistant to join a collaborative primary care team in Stamford, CT. This role provides outpatient care in a supportive practice environment serving a diverse community. Role Scope: • Perform routine physical exams and provide comprehensive primary care services. • Develop medical, educational, and psychosocial care plans for patients. • Manage health concerns within scope of practice, referring to physicians or specialists when appropriate. • Collaborate with supervising physicians and care team members to ensure high-quality patient outcomes. • Provide continuity of care and patient education in a 100% outpatient setting. Schedule & Call: • Weekdays only (Monday – Friday). • Flexible scheduling options available. • No call required. Qualifications: • Unrestricted medical license (or eligibility) in the State of Connecticut. • Graduate of an accredited Nurse Practitioner (Family NP) or Physician Assistant program. • At least 2 years of experience in primary care or a related outpatient setting preferred.
