Responsibilities and Duties
- Work alongside a Physician, with support from an MA or LPN
- Manage a caseload of 15-18 patients per day
- Provide general primary care, chronic disease management, and acute care services
- Conduct follow-ups for emergency department visits and hospital discharges
- Engage in phone-based call responsibilities after the initial two months, averaging 5-10 calls per week
Additional Information
- Must be licensed in New York and enrolled in Medicaid NY
- Clinic experience is required
- Familiarity with EMR systems Epic and Medent will be advantageous
