We’re seeking a Certified Registered Nurse Practitioner (CRNP), Doctor of Nursing Practice (DNP), or Physician Assistant (PA) to join a growing team serving York, Lancaster, and Harrisburg, PA. This role combines virtual visits with in-home house calls—allowing you to practice autonomously, make meaningful connections, and truly impact your local community.

Key Responsibilities

• Deliver high-quality primary care through virtual visits and house calls

• Manage acute and chronic health conditions with a patient-focused approach

• Prioritize virtual triage to ensure timely and effective care

• Travel within a 30 to 60-minute radius for in-person visits

• Maintain clear communication and build strong, lasting patient relationships

Ideal Candidate

• CRNP, DNP, or PA with strong primary care or family medicine experience

• Urgent care or emergency medicine experience is a plus

• Self-motivated and comfortable providing independent care in diverse settings

• Excellent communication skills and commitment to improving patient outcomes

• Passionate about community health and innovative care delivery
