Job Details:
Location: Arizona (with telehealth coverage for AZ, FL, NJ, NY)
Facility Type: Urgent Care
Start Date: May 12, 2025
LOA: 90 days initially, with potential for extension
Provider Type Needed: Physician Assistant specializing in Family Practice
Schedule: 10 hours per day, Monday - Sunday
Split between 2 providers:
First provider: 3 days per week
Second provider: 4 days per week
Responsibilities and Duties:
• Conduct telehealth visits via Facetime or similar platforms
• Perform 10-15 minute consultations with patients
• Assess patient needs and symptoms
• Prescribe medications as necessary
• Provide consultations on oxygen management
• Manage Chronic Care Management (CCM) cases
• Offer home care advice and support
• Maintain accurate and detailed medical records
• Collaborate with other healthcare professionals to ensure comprehensive patient care
Additional Information:
• Medical licenses required for Arizona, Florida, New Jersey, and New York
• Experience in telehealth and CCM is essential
• Comfort with remote patient consultations and virtual care delivery
• Strong skills in family practice and urgent care medicine required
• Ability to manage patients calling from their homes
• Flexibility to work as part of a two-provider team covering the full week
