We have an urgent opening for a Primary Care Provider (NP or PA) at a Health Center in Middleton, WI. This is a first-come, first-serve opportunity!
Details:
- Dates: March 24 - May 30, 2025
- Role: Interim coverage with potential for a permanent position
- Experience: 3+ years in Family Practice preferred
- EMR: EPIC
Services:
- The Health Center supports PCMH, lab, provider dispensing, BH, and PT lines of service. Needing coverage April-May 2025 with possibility to become permanent role if a good fit!
Hours:
- Mon/Tue: 6:30 AM - 4:30 PM
- Wed/Thu: 11:00 AM - 7:00 PM
- Fri: 8:00 AM - 12:00 PM
