JOB DESCRIPTION:
Medical Center in metro Baltimore, Maryland is seeking a Population Health Physician Assistant/Nurse Practitioner to join our team. The Population Health APP will assist in the education and care management for high risk and high need patients. Two years of experience as a practicing PA/NP preferred.
