We are seeking several experienced Physician Assistants to join a large multispecialty group providing house call services throughout the Bronx. This is a permanent, full-time opportunity focused on delivering high-quality care to patients in their homes.
Spanish speaking is preferred, but not required. H1B visa sponsorship available.

Start time: 8:30am until daily patient quota is met

Job Summary
The House Call Physician Assistant provides comprehensive medical care to patients in their homes, focusing primarily on individuals with chronic illnesses, limited mobility, or barriers to accessing traditional healthcare settings. This role emphasizes patient-centered care, strong clinical skills, and effective coordination with interdisciplinary team members.
Responsibilities

Perform comprehensive in-home patient assessments, including physical exams, medical histories, and medication reviews

Diagnose and manage acute and chronic medical conditions

Develop and implement individualized treatment plans

Prescribe medications and monitor therapeutic response

Provide patient and family education on disease management, preventive care, and medication adherence

Coordinate care with physicians, nurses, specialists, and community resources

Conduct follow-up visits and adjust treatment plans as necessary

Identify urgent medical issues and coordinate appropriate escalation of care

Maintain accurate and timely documentation in the electronic health record
Qualifications

Master’s Degree from an accredited Physician Assistant program
-
Active New York State Physician Assistant license
-
Current NCCPA certification
-
Active DEA and New York Controlled Substance registration
-
Post-training clinical experience required (Emergency Medicine, Family Medicine, or Acute Care preferred)

BLS required; ACLS preferred

EMR experience preferred
-
Strong independent clinical judgment and communication skills
Work Environment

Primarily patient homes with varying accessibility and environmental conditions

Travel within a designated geographic area (transportation provided)

Collaboration with a multidisciplinary team
