Primary Care PACE Physician Assistant

Join our robust team as a primary care provider for a San Diego based Program of All-Inclusive Care for the Elderly (PACE), providing high quality, compassionate geriatric care in an outpatient setting. Enjoy lower volume and more time with your own panel of patients!

Opportunity for Loan Repayment or Loan Forgiveness!

Description
-Provide acute, chronic, preventive, curative and rehabilitative medical care to patients
-Manage transitions of care, chronic disease, advanced goals of care discussion and palliative management
-Refer participants to other in-house providers, including behavioral health
-Document visits in the electronic medical record

Hours
-Monday through Friday
-Full time

Location
Vista, California (north county San Diego)

Requirements
• 2 years post completion of clinical training
• Board certification

Preferred
• Employment experience in a community health center
• Experience working with elderly population
• Leadership experience a plus!
