- IM accepted, peds not required.
- Chart closure within 72 hours
- Schedule = 18 PPD, minimum is 18 PPD
- Monday-Friday, 8 or 10 hours shifts possible
- Mix of peds/adults
- Needs experience with Chronic Pain Management
- A decision is typically made within 7-14 days
- APC's with 3 years of experience in similar setting
- BC required
- Active DEA needed before start, and NP Furnishing.
- Credentialing takes 45-90 days
- The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), Covid-19 Vaccine Series and 1 Booster.
