NP or PA – Family Medicine Locum, Eureka, CA (Monday – Friday Clinic Only)
A locum is available for Advanced Practitioners (NP/PA) to support a vacancy in a community health center. Position is estimated to begin January 23, 2023.
Daily schedule is Monday-Friday, 8A-5P, will have 18 scheduled patients per day, and chart closure expectations within 72 hours.
Required: Certified NP/PA Advanced Practitioner with Family Medicine background of 3+ years.
PA’s required to have CA Controlled Substance Education Course Certificate, CA and DEA license.
NPs must have furnishing License and Schedules II and Active DEA.
In addition: before credentialing, the following must be completed - 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), Covid-19 Vaccine Series and 1 Booster
