Post-Acute Care NP/PA – Syracuse
Position Overview
- Role: Nurse Practitioner (NP) or Physician Assistant (PA)
- Setting: Skilled Nursing Facilities (Post-Acute Care)
- Schedule: Full-Time, 5 days per week
- Work Location: In-person
- Compensation Model: Fee-for-service
- Active New York NP or PA License – REQUIRED
- Active DEA – REQUIRED
- Experience in post-acute, acute care, emergency medicine, or clinic settings preferred
- New graduates welcome
- Comprehensive patient assessments (H&P)
- Develop and adjust individualized care plans
- Monitor patient progress and coordinate interdisciplinary care
- Collaborate with physicians, nursing staff, and healthcare professionals
- Educate patients and families
- Maintain compliant and timely documentation
- Structured 3-month onboarding program
