Role Overview-
Location: Oxford, NY

Schedule: Part-time, 2 days per week (Tuesdays & Thursdays)

Setting: Post-acute inpatient / skilled nursing facility

Deliver comprehensive primary care to post-acute patients

Manage both acute and chronic medical conditions

Develop and implement individualized care plans

Collaborate closely with the interdisciplinary team (IDT)

Partner with and work alongside a physician

Experienced clinicians preferred
-
Certified FNP, CNP, AGNP, APRN, or Physician Assistant

Active and unrestricted New York NP or PA license
-
Active DEA license

Productivity-based compensation model (fee-for-service)

Estimated annual compensation: $49,096 – $58,917
