We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our post-acute care team in New London, Connecticut. Deliver high-quality, safe, and cost-effective care in post-acute settings

Provide inpatient rounding in:
-
Skilled Nursing Facilities (SNF)

Inpatient Rehabilitation Facilities (IRF)

Long-Term Care Hospitals (LTACH)

Collaborate with interdisciplinary care teams in accordance with CMS guidelines
-
Coordinate care transitions with hospice, palliative care, and home health agencies

Ensure timely documentation and billing in compliance with Medicare guidelines using gEHRimed EMR
-
Board Certified: FNP, AGACNP, ACNP, AGPCNP, or PA-C
-
Licensed (or eligible for licensure) in Connecticut
-
Experience in post-acute or inpatient settings preferred

Willing to train experienced candidates transitioning into post-acute care

Strong communication skills and collaborative mindset
