JOB SUMMARY
This role supports continuous performance improvement aligned with hospital goals by supervising care coordination staff and ensuring effective patient transitions. Responsibilities include adjusting staffing based on patient needs, mentoring team members, and promoting professional development. The role involves proactive problem-solving, event reporting, and participation in patient care planning to enhance satisfaction and discharge efficiency. Additional duties include managing timekeeping records, conducting evaluations, supporting staff during shortages, and contributing to departmental initiatives and hospital workgroups.

EXPERIENCE AND EDUCATION
Required
- Education
Bachelor's Degree Graduate of an accredited Nursing Program or
Master's Degree Social Work
- Experience
5 years experience as a hospital based RN, or community based social worker. or
3 years as a Social worker or RN Care Coordinator/Case Manager/Navigator with acute care hospital discharge planning experience.
- Licenses and Certifications
(BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA). and
(NLC) NURSING LICENSURE COMPACT-BNE and
(SW) SOCIAL WORKER State of Texas and
Preferred
- Licenses and Certifications
(CCM) CERTIFIED CASE MANAGER or
(ACM) Accredited Case Manager
JOB DUTIES
- Demonstrate continuous performance improvement in alignment with the overall annual Hospital performance improvement targets.
- Directly supervises and evaluates the transition of patient care by other Care Coordination personnel (RN's, SW's, Navigators), as assigned. Participates in regular rounding of staff. Assists with chart audits. Utilizes opportunities to teach and mentor other Care Coordination personnel.
- Reviews staff assignments and revises as necessary to meet the specific needs of the patients/nursing unit. Coordinates staffing with Central Staffing Office/House Supervisor, if indicated. Makes ongoing changes to staffing as determined by fluctuations in census and patient acuity, to assure safe and timely discharges.
- Role models pro-active problem solving by collaborating with staff to ensure effective resolution of identified issues. Utilizes chain of command for unresolved or escalating issues. Advocates for patients and staff to provide for safe and effective discharge planning and transitions of care.
- Demonstrates flexibility and willingness to provide coverage during shifts when staffing crisis/shortage occurs.
- Provides timely, confidential coaching and/or counseling of Care Coordination personnel as appropriate and follows up with accurate reports to Manager and Director. Promotes and enthusiastically supports professional growth of staff by including staff in patient issues/department decisions, hospital committees, and/or continuing education. Assists with maintaining/improving/implementing plans for employee engagement scores.
- Assures first person reporting of events through RL Solutions. Ensures all staff are competent in entering event reports and serves as a resource when necessary. Investigates facts of event reports. Facilitates timely receipt of these reports for follow-up by the Unit Manager. Notifies Nurse Manager/Risk Manager immediately of high risk situations.
- Actively participates in routine and special patient care conferences, rounds, etc., as indicated. Responsible for maintaining/improving/implementing plans for patient satisfaction scores. Works with Manager and other disciplines to streamline discharge of patients and reach goals for timely to a discharge from time DC orders are written.
- Provides input and/or conducts annual performance reviews and orientation evaluation of personnel assigned to the unit.
- Maintains Kronos records on a daily basis, monitoring for trends that require intervention i.e., late clock-ins, incremental overtime, missed punches.
- Assists with staff meetings, special projects and the development, implementation and evaluation of performance improvement activities. Actively participates on hospital work groups.
