Summary

Under the guidance of and direction of the Care Management clinical leadership and as an extension of the interdisciplinary team, the Case Management Assistant (CMA) actively participates and assists with the patient and caregivers bedside assessment and coordination of post-acute resources in line with the established plan of care. Primary responsibility of this position is ensuring patients receive post-acute care from quality and valued providers while taking the lead in the coordination of follow up appointments. The CMA assists in the readies of the patient for discharge, participates in the communication to the patient and caregiver on the post-acute plan, assists with preference based on quality, leads the referral process and provides resources to patients as directed by the Care Manager. The CMA participates in the delivery and acts as a resource in compliance and regulatory requirements as part of the Conditions of Participation.

Requirements

Education:

High School Diploma or equivalent required.

Associates or Medical Assistant degree preferred.

Experience:

1 year of previous clerical or secretarial experience required.

Hospital or Post Acute experience preferred.

License: N/A

Other:

Basic knowledge of Microsoft Office.

Excellent communication and customer service skills.

Ability to multitask responsibilities.

Calm, professional demeanor .
