An outpatient Cardiology Nurse Practitioner is sought to join a specialized Heart Failure Program within a community-based ambulatory care hub. This part-time position (24 hours per week) consists of 8-hour shifts and focuses on delivering advanced clinical care to patients with heart failure in an outpatient setting.
This role collaborates closely with cardiologists, primary care providers, nurses, and interdisciplinary healthcare professionals to manage complex heart failure cases. The position emphasizes optimization of therapy, reduction of hospital readmissions, and improvement of patient outcomes and quality of life.

The program focuses on early symptom recognition and prevention of avoidable hospitalizations. The team offers rapid response to acute concerns through telephonic medication management, same-day urgent clinic visits including provider evaluation and on-site IV diuretic therapy, and mobile integrated health services with in-home paramedic visits for IV therapy when appropriate. Qualifications
* Graduate of an accredited Physician Assistant or Nurse Practitioner program
* Current licensure as a Physician Assistant or Nurse Practitioner in the Commonwealth of Massachusetts
* Prior experience in advanced heart failure or acute care preferred
* Current CPR certification required
* Strong team-oriented mindset with enthusiasm for interdisciplinary collaboration and delivering exceptional patient satisfaction
