We are seeking an advanced practice clinician to join a growing post-acute care team in the Syracuse, New York area.

In this full-time role, you’ll provide cardiology consults to adult patients in skilled nursing facilities. With no after-hours or weekend call, you’ll enjoy a consistent Monday–Friday schedule that allows you to focus on patient care and maintain work-life balance.

What You’ll Do:

  • Conduct cardiology consults in post-acute care settings

  • Collaborate with facility staff and interdisciplinary teams

  • Monitor and manage chronic cardiac conditions in partnership with supervising physicians

  • Contribute to quality care initiatives and clinical improvement efforts

  • Document using a mobile EMR system, with full training and support provided

Who We’re Looking For:

  • Nurse practitioners or physician assistants with an active New York license

  • Active DEA license (or ability to obtain)

  • At least 2 years of experience in cardiology or post-acute care preferred

  • Strong communication skills and a collaborative, patient-centered mindset

  • New graduates with relevant training and a passion for cardiology are encouraged to apply—additional training is available
